Laparoscopic Repair of Morgagni Hernia: Case in Adult Patient

Abstract
Hernia of Morgagni was first described by Giovanni Battista Morgagni, an Italian anatomist and pathologist, in 1769. Hernia of Morgagni is located just posterolateral to the sternum. It has also been called retrosternal, parasternal, substernal, and subcostosternal. It is caused by a congenital defect in the fusion of septum transverses of the diaphragm and the costal arches. This weakness in the diaphragm would later be stretched by a rapid rise in the intraperitoneal pressure, giving rise to a hernia. Hernia of Morgagni is the rarest type of congenital diaphragmatic hernia. Here, we report a case of Morgagni hernia in an 89-year-old woman, who was suffering from vomiting and constant epigastric discomfort for a year. Chest x-ray showed an unusual bowel gas pattern on Rt. chest wall, and atelectasis on both low lobes. On Abdomen and chest CT, herniated bowel loop was observed, as well as mesenteric fat at the anterior aspect of the Rt. hemithorax. We performed a laparoscopic operation and successfully reduced the contents, and the sac was done with mesh closure of the large defect. Chest tube insert was also done. Patient was discharged at postoperative day 7, without any other complication.
